Special Population Registry
The Special Population Registry provides vital information to emergency responders in the event of a 9-1-1 call and/or during a widespread disaster (e.g. hurricane, flood, blizzard, power outage, disease outbreak). This program is voluntary and individuals on the registry have the option to accept or deny assistance. Completion of this form in no way guarantees that the registered individual will receive immediate or preferential treatment in a disaster.

Kidbase Registry
Lincoln County Emergency Medical Services (EMS) has recently started participating in Kidbase. It is a program for children with chronic medical conditions and/or special needs. Kidbase allows EMS and area hospitals to quickly and better serve your child in the event of an emergency. To participate in this FREE program, begin by clicking the link provided. This electronic form will ask you for some basic information. Once your information is received by Lincoln County EMS, our Kidbase Coordinator, Paramedic Ashley Richard or another team member will contact you. Our team member will schedule a date and time for a home visit so that she can perform a brief assessment of your child. She will then assist you in completion of the Kidbase form. This form will remain on the refrigerator of the home where the child resides. Additional forms will be kept by your child’s caregivers or others who spend a significant amount of time with your child. In the event of an emergency, this information will be available to the medical providers who will be assisting your child. Kidbase helps take the anxiety out of an emergency, speeds up the treatment process, and helps alleviate errors. We are very excited to offer this program!
