The Lincoln County Health Department invites you to participate in the 2016 Community Health Opinion Survey! Every three years, health departments are required to complete a Community Health Assessment (CHA) Report.
The CHA helps to identify data trends, issues, barriers, and community strengths regarding the health of our county. The Community Health Opinion Survey is conducted to help categorize community concerns and determine priority focus areas for the years following the most recent CHA. We will use the results of the survey to help address health and community concerns in our county. We need input from all Lincoln County residents in order to draw accurate conclusions from the survey results.
To take part in the survey please type the following Web-link in to your computers browser and follow instructions. If you would rather take a paper survey, they are available for pickup at the locations below until September 30, 2016. Please come by during normal business hours to get your copy! Please return all paper surveys to one of the locations below by September 30, 2016.
Your answers to all survey questions will be completely confidential and cannot be linked to you in any way. The survey should take no longer than 15 minutes to complete!
